Applicants must have
participated in the Foothills Soccer Club program for a minimum of 1
year.
Application Procedures
Applications should be completed once the player has committed to the
team by completing all commitment forms for the Foothills Soccer Club.
The application can be returned to your team manager, coach or mailed
to: Foothills Soccer Club, PO Box 15, Youngwood, Pa. 15697.
All material becomes property
of the Foothills Soccer Club.
Only one application per
playing year will be accepted
