FSC Multiple Player Family Discount Form

The discount will offer a $100.00 per player on the Foothills U-12 to U-18 teams.

The discount will be applied to the oldest player on the Foothills team Budget.

The managers will be informed of the discount once a completed form is received.

In order to receive the discount players and parent agree to the following:

Complete the on line form
Agree that players participate in all team budgeted functions
Agree that their Foothills teams are the number one soccer priority other than ODP for all players listed on the discount form.
Family has not received a Foothills hardship payment

If the player and parents fail to agree and participate in items 1,2, and 3 the discount is void and the full budget amount must be paid to the team by May 1 of the playing session.
